App randomly skipped a batter...

Ok, I did a search and found a lot of instances where people asked HOW to skip a batter, but none that were complaining of the app skipping a batter without recording it or being prompted to.
Last night we were scoring a game when the app randomly turned the lineup over, skipping the final batters first at-bat. We didn't catch this until the middle of the following inning when we noticed that the wrong person was batting. We tried to go back and fix it, but no matter what, the app kept skipping whoever was in that lineup location for the first at bat. So we pushed that guy (batter #10) to the top of the order so that all the remaining at bats would line up with their respective player, and only the first 2 innings would be off. After that initial problem, the app scored properly and all 10 batters were given their at-bats.
So after the game, I went back to see if I could figure out where we had gone wrong, but I couldn't find anything. There were no substitutions, no skipped batters, nothing that could indicate that the 10th batter had been skipped... it just went straight from #9 to #1. Could I get some help shifting the at-bats around to the proper individuals?
