Problem Changing the Pitcher

I was scoring the Rangers / Diamondbacks game today on the radio, and when they made the first pitching substitution, I did what I thought was the right procedure. Hold down number on the pitcher's mound, it brings up the roster, and lets me sub in the player. That worked. Or so I thought. I saw the new pitcher's name (Michael Kirkman replacing Colby Lewis).
Then I noticed that while Kirkman was shown as being on the mound, he wasn't in the lineup above (see the red circles). I thought that was odd, so I manually went in there and replaced the pitcher, but then I noticed that my pitcher was Kirkman from the start of the game!
GAH! What happened here? Since this was radio, I had no facility to pause the game to fix the problem, so I had to stop scoring the game. Why did this happen? Did I miss something with the proper procedure to substitute a pitcher or something?
