There is plenty of time to do defense and really as long as you do one kid, you can always go back to Play by Play and change everyone you need to.

I'd like to simply add, that in youth games, there often isn't enough time to get it all at the start of the inning. With umpires operating under game time limits and trained in game management to "keep the game moving", innings can start pretty fast. While I agree you can get a few substitutions in if the kids do happen to "turn their back your way", the ones that don't you can't capture before the plays start. So you end up catching up on the defensive posistions in between the action when players happen to turn your way and show their back...in fact the plays force them to turn as they field the ball and play. 
As for going back to play by play, which usually happens after the inning (and the action, which can be a little odd and furious in kids ball - with odd play scenarious due to "youthful errors" and lack of attention...I've seen scoring scenarious where we are all scratching our heads...iScore does ok once you figure out what just happened...really what just happned, I've never seen that!), how do you know where everyone was defensively? -- unless you have a pad of paper and were taking manual notes to go back and enter after the fact in "Play by Play" at a later time?
It's still easiest to capture data as you see it happen (the whole point of scoring) and unfortuntely, identifying players "happens" throughout the inning as they expose their numbers.
So I am with the original suggestion of a sort of "dual sided button switch" when you tap on the definsive posistion for subsitution that maybe defaults to "standard substitution" (Sub), but you can tap on "retoactive to beginning of inning" (Retro). Something like like that would be great and I'm all for that. A selector like you have in player entry in the roster for "Bats - Left/Right" or "Throws - Left/Right".
