Two problems with courtesy runners

I've run into a couple issues the past couple games with courtesy runners.
In the first case, I just missed the entry of the courtesy runner. I tried to edit her in on the play-by-play substitutions, but it would only let me substitute her into the game for the pitcher. No problem, I thought. I'll do that and take her out when the team goes on defense. But it actually continued to credit the pitcher whom she was running for with the advancement around the bases and the eventual run scored.
In the second case, the courtesy runner wasn't on the team's roster in the program when I created the game. I added her later, but she wasn't then available to me when I needed to put her in the game as the courtesy runner. There didn't seem to be a way to add her to the roster on the fly, if you will. In this case, she was only going to be a runner in this game because she was nursing an arm injury.
