I am trying to "clean up" my mistake when I registered last year when I included age in URL. I created and paid for a new team site today. All game stats and data shows on new site but when I'm in team manager in app it shows old team site with a valid to date on 05/06/2013. How do I have the correct site linked to the app?

Log in to your Admin Website, and change the old website to show the data for a different team on your device (any different team). You can follow the instructions int eh following link from our FAQ to change the team a website is linked to:

Along this line. I created a website for a team in April and stupidly used the age in the name. Is there a way to change the website name?

I have not found a way to change the URL without buying a new annual subscription for 20$.
the url structure is iscorebaseball.com/ XXXXXXXX or iscoresoftball.com/XXXXXXXXX
where XXXXXX is the name you define at subscription purchase
If you can live with the dated URL till it expires, per the prior post you can assign a new team to it through the admin web site (my.iscorecentral.com).
if it really bothers you, you can buy a new subscription for 20$ and assign it the new team.
