The training video at http://iscoresports.com/baseball/training.php" onclick="window.open(this.href);return false; goes over making substitutions. When the team takes the field, you just tap any fielder that is wrong, and select the right one. Should take about 30 seconds even to change an entire defensive lineup.

Re: Changing Field positions with each inning
If you know in advance (pregame) what positions the players are going to be playing each inning, you can enter their defensive pre-sets when you create the game.
Once you have selected your options for the game itself and clicked "Done", you come to the Game Info screen. Click "Play" in the upper ight corner and it brings you to the line-ups for each team. Using the three horizontal lines to the far right of each player's line, you can move them up or down so they are in the right batting order. In the bottom right, click the options button. You can select "Pre-set Fielders." That should get most of your players in the right positions each inning so you aren't having to make nine substitutions every half/full inning. You can still do the substitutions that FTMSupport suggested if things change on the fly.
