In fastpitch, many times it's 2nd and SS that end up covering the corners.

If the umpire feels that the slide was intentional to break up a double play, then yes, two can be called out.

I found this powerpoint presentation for umpires that does a good job of explaining subs, courtesy runners, and dp/flex. It says it is for ASA and HS.
